Most of the ranks are automated by Nightbot and StreamElements, except for the Regulars rank, that needs to be managed manually.
Higher ranks allow the viewers to have access to extra chat commands, unlock more chat activities, as well as the ability to participate in raffles/giveaways.

Being part of the Regulars rank requires you to be in the channel regularly, as well as being a somewhat active member of the chat.
The benifits of this rank are mostly the extra chat commands so, if you're a lurker, you don't have much to gain from being in it.
